I, hereby give my approval for my child’s participation in the youth soccer camp prepared by Odenton Soccer Academy. In exchange for the acceptance of said child’s candidacy by Odenton Soccer Academy., I assume all risk and hazards incidental to the conduct of the activities, and release, absolve and hold harmless Odenton Soccer Academy and all its respective officers, agents, and representatives from all liability for injuries to said child arising out of traveling to, participating in, or returning from selected camp sessions. In case of injury to said child, I hereby refrain from initiating a false or lawsuit against Odenton Soccer Academy, including all coaches and affiliates, all participants, sponsoring agencies, advertisers, and, if applicable, owners and lessors of premises used to conduct the event. Because the risk of being injured is inherent in all sports activities, including soccer, "Odenton Soccer Academy" activities are covered by a general liability design to cover potential medical expenses, bodily injuries, and facility damage that might occur only during our activities and under specific circumstances. Some of sports injuries include, but are not limited to, the risk of fracture and concussion.
